both the student and Line Manager should fill out a copy of this form, then prepare 2 further copies; for each other and one for the course tutor. To maximise the benefits of this review, it is important that the tutor recieves their copy before the meeting, to allow them time to prepare properly.
The Practice Handbook contains the 'Portrait of a Worker' information, which should give you some guidance on the qualities and skills required for each level of competence. Please note that the NOVICE grade is a fail, and students in 2nd and 3rd year are not permitted to proceed to the next stage of the course if they have Novice gradings in any area.
The cover sheet is designed to be filled in at the conclusion of the 3-way meeting between line manager, student and course tutor. This will then represent the student's personal development action plan for the coming year.
This page contains the forms you will require to complete the Annual Review Form for each year of the BAYAT course. Further guidance on how to use these forms is contained within the Practice Handbook.
PLEASE NOTE - the same sheet is now used for all levels. Competences which are not yet assessed can simply be left blank.
